If you are a Senior Music Student and have participated in any fundraisers during high school, you have the opportunity to receive a payout of your DSF account. In order to do so, please complete the form (see link below) and email a scanned copy to mpa.shawna@gmail.com. Your balance will be checked, and if you have a balance, you will receive a response via email. Please follow all directions and provide all requested information.
